The world of the primary care doctor has been rapidly changing and will continue to do so over
the coming years. Insurance concerns, higher patient expectations about care, an expansion in services provided and a decline in reimbursements are all prompting the need for primary care physicians to find new ways to stay successful and grow their practices as a result. Being a primary care doctor is not easy and I applaud those who follow their calling in the face of these and other health care challenges.

Unpaid time is unfair to doctors

I have a young son who asked me the other day why Nat, our CEO, paid me for my work. This caught me by surprise and I asked what had prompted the question. He told me that he understood why construction workers got paid for building roads, why firefighters got paid for putting out fires and why NHL hockey players got paid for scoring goals (something I struggle to understand sometimes myself).

Last week’s landmark decision from the Supreme Court clears the way for full implementation of the 906-page healthcare reform law. Know anyone that read it from cover to cover? I didn’t think so. No worries - basically, the law establishes insurance exchanges in each state, prohibits insurance companies from discriminating against the sick, and requires nearly all Americans to prove that they carry health insurance starting in 2014.

As we pause on July 4th to reflect on the spirit and sacrifice that created and sustains the independence of our great nation, the independence of the primary care practice is deserving of reflection and celebration as well.

Our nation’s primary care doctors are on the frontlines of our health care system caring for our families, young and old. They are our health care “home base” providing a continuity of care that can be measured in decades and not just in years. They help us navigate through complex chronic health issues and sometimes an equally complex health care system.

The traditional way of thinking about your technology Return on Investment (ROI) is in strictly financial terms: Take the cost of a technology investment and attempt to quantify the financial benefits as a percentage of the cost. Match the cost and you’ve broken even. Generate a financial return greater than the cost and you’re dollars ahead.

But let’s face it. Much of the time, the costs included in a ROI don’t fully reflect the total cost of ownership: the initial purchase cost, training, implementation, support, upgrades, and maintaining regulatory compliance—to name just a few.
